Health Certificates

Traveling across state or international borders necessitates a Certificate of Veterinary Inspection (CVI), also known as a Health Certificate. In order to prevent the spread of animal diseases across state lines, state departments of agriculture and other state agencies have created rules and regulations which govern the importation of livestock, companion animals, equines, and other animals. All states have set forth stringent requirements for the importation and movement of livestock and companion animals. These requirements are enforced by the state boards of animal health and the USDA.

To help decrease the possibility of mistakes and increase efficiency, we ask that you complete the appropriate form if you are in need of a Health Certificate.

Please observe the following guidelines:

  • All inter-state Health Certificate requests must be submitted a minimum of 3 business days before they need to be picked up.
  • All international Health Certificate requests must be submitted a minimum of 10 business days before they need to be picked up.
  • Please complete ALL applicable tables. Blank tables will have a direct impact on when the Health Certificate will be available to be picked up.
  • Please provide ALL show/exhibition/sale vaccine requirements in the Additional Statements table (a link to the show/exhibition/sale website is not acceptable.) Incomplete/inaccurate information will have a direct impact on when the Health Certificate will be available to be picked up.
  • Please provide ALL show/exhibition/sale statement requirements in the Additional Statements table (a link to the show/exhibition/sale website is not acceptable.) Incomplete/inaccurate information will have a direct impact on when the Health Certificate will be available to be picked up.
